I understand that, as a non-employee of Ascension Lutheran School, neither I, nor my vehicle, nor its registered owner is in any way covered by the insurance of Ascension Lutheran School. I further understand that Ascension Lutheran School in no way assumes responsibility for damages or injuries resulting from the operation of the vehicle I will be driving on the trip identified above. (If I am an employee, I understand that I shall be indemnified by Ascension Lutheran School as provided by law.)
CERTIFICATION OF INSURANCE
I hereby certify that I am insured with at least the following coverage of $100,000 Bodily Injury, 1 person; $300,000 Bodily Injury, 2 or more persons; and $50,000 Property Damage. I also certify that I hold a California Driver's License that will be valid when this trip is taken. I agree to provide copies of my driver's license and proof of insurance to Ascension Lutheran School upon request.
I understand that as a driver for this field trip I:
Must provide individual seat belts for each passenger
Cannot make any stops between the school and our destination and back except for emergency purposes (e.g., stopping for refreshments of any kind is not permitted.)
May not view my mobile device while driving, nor send or read text messages while driving, nor place or receive non-hands-free telephone calls while driving
Shall abide by all traffic laws while driving
Am considered a chaperone and I will have responsibilities of student supervision and well-being during the field trip.
FROM STUDENT/PARENT HANDBOOK FAMILY CODE OF CONDUCT: All Ascension Lutheran School students and caregivers (parents) will strive to abide by the Code of Conduct at all school and church functions through following the behavioral expectations:

Not engaging in relational, online or physical bullying, discriminatory, intimidating conduct or innuendo, sexual comments, and sexual harassment of others
Using only respectful appropriate language - profanity, foul language, degrading comments, put downs-or-jokes - are not acceptable and may be considered harassment
Respecting the authority of the school to address any incident that occurs at school. Caregivers/Parents should share their concerns with the school personnel and allow them to facilitate the follow-up
